After a hospital stay, the first 72 hours are critical. During those important three days and beyond, our trained caregivers can support you or your loved one through a successful recovery at home.
Feel better and heal better—faster.
Following surgery or a hospitalization, most people are more comfortable at home in their own bed, surrounded by the things they know and love. Our hospital-to-home care solution is comprehensive and flexible, giving you the physical and emotional support needed to recover safely and comfortably. We handle everything required for a successful recovery, so you can relax, rest, and focus on getting better.
We understand post-hospitalization care needs.
Hospital discharge is something to celebrate but it can also be stressful, overwhelming, and even risky without the right support.
We can reduce the stress of recovery—and the risk of readmission—by ensuring a smooth transition from the hospital, making sure the home environment is safe, and providing the help and medication reminders needed to heal quickly and comfortably.
We’ll develop a Care Plan that sets you up for a successful recovery—and helps relieve family members’ fatigue and worry. We’re also experienced coordinating care with healthcare teams, physical therapists, and others to avoid setbacks and support ongoing health in the future.

How we can help.
Our caregivers have experience helping clients during their cancer treatments and recovery from surgery as well as with palliative care. We can provide transportation and companionship for medical appointments, chemotherapy, radiation, or other treatments, prepare tasty, nutritious meals to combat lack of appetite, provide mobility support and help with bathing when fatigued, and keep the house safe and tidy. And, if you’re feeling overwhelmed or down about the prognosis, we’ll be a source of support as you work through next steps.
