Who’s On Your Care Team

Your Client Success Manager

You'll have a dedicated Client Success Manager who leads your Care Team and serves as your single point of contact. Your Client Success Manager works directly with you, overseeing your Care Plan, keeping you informed about any changes, addressing concerns before they escalate, and ensuring your ongoing satisfaction.

Your Client Success Manager serves as your advocate: someone who knows your family's unique situation, coordinates with all the moving pieces, and makes sure nothing falls through the cracks. Our Client Success Managers are deeply knowledgeable in all aspects of aging and care, experienced problem solvers, and available 24/7 when needed.

What Your Client Success Manager Handles

  • Creating and updating your personalized care plan
  • Communicating with your physicians (only in areas offering skilled nursing services)
  • Proactively identifying and addressing changing needs
  • Serving as your primary point of contact for questions or concerns
  • Ensuring seamless communication between all team members
  • Providing guidance on care decisions and transitions

Your Caregiver Manager

Your Caregiver Manager expertly matches you with the right caregivers based on your specific needs, personality, and preferences, not just who's available. They work closely with your Client Success Manager to ensure consistency and quality, handle all scheduling and caregiver supervision, and coordinate the day-to-day logistics of your care.

Caregiver Managers are also available 24/7 to support both caregivers and clients if challenges arise. If your regular caregiver is sick, they handle the replacement. If care needs adjustment, they make it happen. You never have to worry about coverage or coordination—that's their job.
